It's mostly about relative proportions. You can have broad joints with a narrow ankle (like me), or a wide foot with a low instep.
Yes, it is true that bespoke vs MTM will not make a significant difference. The main advantage of bespoke will be in style and make, such as hand sewing, tapered heels and so forth.
But it doesn't matter if the product is bespoke or MTM. What matters most is who does the measuring and fitting and high quality men height increasing shoes ladies how easily they can be reached. It's important to find someone who has experience with the system, or who has worked with the lastmaker over time, and can be contacted with any questions or concerns.

The same question has been bothering me for some time. Going to a bespoke manufacturer vs. MTM London. GG suggested that you start with mtm. Many RTW shoes have too wide heel cups. Staff at GG high quality men height increasing shoes ladies stated that the heel cups on their shoes are narrower by default. This I found to be true compared with EGs shoes. However, I am still not sure.

It's okay. Separate MTM from MTO, however. If they aren't changing the last, you can think of it as MTO. This is just a ready-made shoe that's not in stock.

I like the look of these shoes. I suppose that would make me old-fashioned.

They look great to me. They don't have fancy bespoke shoes like the slim welt and angled heel stack, but I find these Height Increasing Shoes
